Note: The medical form and waiver both need to be mailed to the camp office BEFORE campers can be added to our roster (see forms below). The deposit may be mailed OR paid online. Please see our Payments page (on the drop-down menu) to pay online.

Please write your camper's name on the check and your correct address if different from the printed address.

Please note that ALL forms (waiver and medical/emergency form) must be received before your application will be processed. Campers will be admitted and given audition times in the order in which their applications are received. The deadline for applications is May 12. We do accept campers after this date if we still have openings (check with the camp office).

Please note: Our camp fills up VERY quickly! In order to be assured of a place, register for camp and mail all forms and deposit as soon as possible! Campers will be placed on a waiting list once our enrollment is full.
